1. How do I know that I need to complete a survey?
If you are invited to complete a survey you will receive a letter from DHA explaining what the survey is about and how to complete one. If you do not receive a letter with this information please do not complete a survey.
2. I have received a reminder letter but have already returned my survey.
If you have returned your survey but still received a reminder, please disregard the letter and do not complete a second survey. We try to minimise this happening but occasionally early returns are not processed in time and a letter is sent.

4. What is my UPRN?
The UPRN is the Unique Property Reference number that is allocated to your property. Each DHA property has a UPRN.
5. Where is my survey ID number?
Your survey ID number is located in the top right hand corner of the cover letter that was included in the mail pack. If you have misplaced the cover letter you may enter either your PMKeys number (for ADF tenants) or UPRN (DHA lessors).
6. I wish to provide additional feedback.
DHA values the feedback of tenants and lessors. Our surveys are designed to collect specific information, however, your comments on any aspect of our service or products are always welcome. Should you wish to offer additional feedback, please contact your local Housing Management Centre on 139 DHA (139 342). For more information on providing feedback please refer to your feedback.
7. My personal details are incorrect.
If the address or personal details on your survey pack are incorrect please contact DHA on 139 DHA (139 342) to correct this information.
8. Who can fill out the survey?
My partner is away can I fill in the survey?
Partners or spouses of ADF members are able to complete the Annual Tenant, Maintenance and Housing surveys on their behalf.
If your partner has been sent a survey for lessors, you may complete the survey if you are also a lessor of the same property.
9. I am in a private rental property receiving rent allowance – should I have received a survey?
Although you are living in a private rental property on rent allowance, you may still have been sent the Annual Tenant Survey. Your feedback will help us to compare our Service Residences with properties in the private rental market.
10. I have just discovered a survey that I have not completed and returned – should I still complete it?
Yes, If you can complete and return the survey before the closing date specified in the cover letter or on the web page please do so. If it has passed the closing date please check the web page to see if returns are still being accepted.
11. I am a lessor and own a number of properties, which property should I refer to?
The property we are asking you about is identified in the cover letter which was included in your survey pack. Please respond to the questions in the survey based on your experiences with DHA regarding that property.
